GWH staff have announced the launch of the new Swindon Integrated Care Alliance Coordination Centre.
The start of this month has marked the launch of GWH's new, 'first ever' care coordination centre in Swindon.
The new centre aims to physically integrate colleagues from across the GWH NHS Foundation Trust's whole health and care system, meaning it will help to better coordinate its patients’ care, from the moment they dial 111, 999 or as they walk through the doors to the hospital.
The centre is made up of clinical staff, therapists, social care, voluntary services, transport, mental health services and more.
Dr Timea Novak, Deputy Associate Medical Director for Integrated and Community Services and Lead for the Navigation Hub shares what she is most looking forward to and the ways in which the centre will benefit her colleagues and patients.
